Case 3.

A 56 year old male was brought to the emergency room after collapsing in the street.  After regaining consciousness, the following was noted upon examination:

  1. complete loss of epicritic sensations (fine touch, localization) on the left side of the body - these were slightly diminished on the left side of the face;
  2. protopathic (crude pain, temperature) sensations were barely perceived on the left side of his body;
  3. a left spastic hemiplegia with increased deep tendon reflexes and Babinski sign were seen;
  4. the patient could wrinkle his forehead, but the left side of his mouth drooped;
  5. a left homonymous hemianopsia was observed.
